Collecting the information required for the Care Passport is probably the most demanding task when creating them for the first time.  

Collecting the data
The questionnaire is has to gather data for all the sections and topics in the Care Passport.

Quality of the data
Careful design of the questionnaire is critical to the get quality and accuracy of data needed.

Recording
The Questionaires are must be filled in by appropriate members of the person's support team.

Vetting & Checking
The entries must be very carefully checked by the person responsible for supervising the person's care.

Filling in the Quetionnaire
The above task may seem to be fairly simple but the the Care Industry employs a very wide variety of people, many have other languages as their mother tongue and may find it challenging to both understand the questionnaire and complete it in English.  

Transcribing the data
When entering the data on the computer from the completed forms interpreting the information can be an arduous and time consuming task.  If answers are unclear great care and consideration is required when interviewing staff on the answers they have given as many can be very sensitive about their literary skills, after all they are employed primarily for their caring abilities.
